Records Request
All record requests by educational institutions and other entities for former students of the PHSD must be submitted electronically utilizing our new student record request system in order to provide an improved and more secure service for our transfer students.
Fax, phone, email, and postal mail requests will no longer be accepted.
The new K-12 Transfer app is a secure, self-service software that requires the following steps:
